When you choose Fair Haven Community Health Center

You join a health care team when you choose to receive your healthcare at FHCHC.  We work together in partnership to provide you with the high quality healthcare you and your family needs. When you decide to join our team, you can choose your Primary Care Provider (PCP) and a team of healthcare providers who work best with you. Your team will help you coordinate care across many healthcare settings such as hospitals and specialty care providers. We are committed to providing the care and support you need when you need it, and helping you learn how to manage your own care. Schedule an Appointment

To schedule an appointment, please call (203) 777-7411.

If you are a new patient, be prepared to fill out a registration form before your visit. If applicable, please come prepared with your health insurance card and medications you need refilled to every appointment.

Please bring the following to your first appointment:

·         Proof of address

·         Photo Identification

·         Social security number, if applicable

If you do not have insurance:

·         Proof of family and/or individual income such as pay stubs, employer letter, SSI/Disability benefits letter, unemployment benefit notification letter, W-2 or 1040 tax return form

·         If you are unemployed and have no income source, we will ask you to complete a form about your financial situation.

Interpreter and translation services are available to patients who do not speak English.


FHCHC is a federally-qualified health center (FQHC) and provides care regardless of your ability to pay.


Co-payments are expected at the time of service.  

We accept all types of payment: cash, check, debit, and credit card.

We accept all insurances, including Medicare, Medicaid (Title 19), HUSKY and many private insurance plans.  If you do not have health insurance, please make an appointment with our Certified Application Counselor at FHCHC.

We offer a sliding fee scale for patients that are not eligible for health insurance. Staff is available to discuss payment options if you cannot afford your care. 


Late or Cancelled Appointment

Please arrive 15 minutes before your appointment time so that you have enough time to register.  If you are late, we cannot guarantee the length of your visit.

We understand there are situations when it is necessary to cancel your appointment. You are responsible for calling the health to cancel your appointment. This allows us to offer your appointment time to someone else in need.

Repeated cancellations or “no-shows” may result in termination of service .  If you have missed an appointment, please call us immediately. We want to know you are safe.  If a health care provider calls you to ask about a no-show or cancellation, please return the call.

*There are times when your health care provider needs to cancel and reschedule an appointment. We will make every effort possible to notify you as soon as possible. Please be sure you have updated your contact information at FHCHC so we can reach you.* 

After Hours Care

In case of a life threatening emergency, please dial 911 or proceed to the nearest Emergency Room.  

In case of a non-life threatening medical event, please dial the main clinic number (203) 777-7411. Our answering service provides 24-hour telephone service to our registered patients. Please leave your name, clinic number, and telephone number with the answering service. Speak clearly and stay by your phone. A clinician will return your call. Please call again and leave a message if a clinician does not return your call within 30 minutes. 

In the event that you are hospitalized, our clinicians will monitor your care at the Yale New Haven Hospital.
