Fair Haven Community Health Care (FHCHC) has been recognized for the 4th year in a row as a national Health Center Quality Leader. This is the highest level of quality leadership awarded by the Health Resources & Services Administration (HRSA) among the 1,400 health centers across the country. Just as school grades give parents feedback about their child’s academic progress, Health Center quality scores inform you and your family about the best places to receive medical care.

FHCHC is the only health center in New Haven and one of only three in Connecticut to be designated a 2020 Gold Award Health Center Quality Leader for best overall clinical performance. FHCHC ranked among the top 10% of Health Centers nationally on indicators such as perinatal health, preventive health screening and chronic disease management for adults and children.

In addition, FHCHC’s Behavioral Health Care was recognized in the top 1-2% of all health centers nationally. This focuses on quality measures that promote mental health such as screening and treatment for depression. Fair Haven’s behavioral health services are available at five convenient locations in the New Haven area. FHCHC was also recognized for leadership in Advancing Health Information Technology (HIT) in areas such as telehealth and patient care. FHCHC has grown its telehealth program dramatically, increasing access to care during the COVID-19 pandemic.

As Federally Qualified Health Center, FHCHC is committed to the community’s needs and to bringing high quality clinical services to all patients, regardless of ability to pay. This includes making free COVID-19 testing easy, safe, confidential and accessible to everyone. Partnering with State of Connecticut, FHCHC is offering free COVID testing at the 374 Grand Avenue, New Haven location from 9am-5pm Monday-Friday. Testing is available to everyone by calling 203-871-4179 for an appointment. “Our free testing program makes it possible for everyone to be tested, as we all do our part to keep our families and communities safe,” added Dr. Lagarde.
