[vc_row][vc_column][vc_column_text]In the aftermath of Hurricane Maria, Fair Haven Community Health Care established a program for dislocated PR residents new to the New Haven area. Leveraging an approach of single appointment care visits, a broad variety of services were  provided to help individuals and families experiencing the upheaval of resettlement to Connecticut.  Each patient visit included access to health care, insurance coverage transfers/activation, prescription refills, behavioral health and PTSD counseling, and care coordination to address housing, transportation, employment and education.

Born out of the commitment of Fair Haven staff who had friends and family affected by Maria, the Displaced Persons Clinic was seen as one small attempt to lessen the burden new arrivals from Puerto Rico were experiencing once here in New Haven. What made the program even more impactful was our partnership with community leader JUNTA for Progressive Action, who worked with us on many levels to help spread the word and provide access to even more dislocated individuals who could benefit from the clinics.
