Each organization has a long history of excellence in their medical service areas and is now bringing accessible health care further east on the Shoreline, complimenting our East Haven/Trolley Square location.
Shoreline Family Health Care provides is perfect for patients of all ages who are seeking a primary care provider or a behavior health provider. The location offers an on-site lab and pharmacy as well. Patients can also take advantage of our integrated care model, offering patients with certain health needs access to both primary and behavioral health care providers within the same office visit.
