[vc_row][vc_column][vc_column_text]Formally launched in July 2018, Fair Haven Community Health Care’s Population Health program was conceived as a way to target patient groups who might benefit from specialized care clinics that integrate medical care and care coordination services. These unique programs offer patients resources and focused care integration, allowing providers to identify the root cause of medical concerns and truly understand and treat the whole patient, not just the symptom.
To date, we offer in-depth, patient support through programs for Diabetes, Asthma, Ambulatory Care, and Pediatric Developmental Behavior, with additional focus areas in the works.
