We are delighted to announce that Fair Haven Community Health Care has been awarded additional funding from the Health Resources and Services Administration (HRSA), this time to support increased access to services for substance use disorder and mental health programs. Of the HRSA $352MM awarded nationally, FHCHC received $327,750 to expand services for those with substance use disorder and to address growing community needs.

Dr. Suzanne Lagarde, our CEO, highlighted the importance of this funding. “This grant will help us expand services to all those in need. Our substance use disorder program uses an integrated framework for patient care, drawing from a full complement of care resources. This approach allows us to integrate the expertise of our internal medicine and addiction/behavioral health providers, as well as our care coordination team and provide seamless, coordinated care for our patients. As we expand this program, we continue to evaluate it through a health equity lens, to ensure equitable health care for all.”

Patients or family members interested in learning more about FHCHC’s integrated care program should call Maya at 203.809.3511.
