The History of Fair Haven Community Health Care

Our Journey From Humble Beginnings to a Community Health Leader

Since opening our doors in 1971 at Columbus School on Grand Avenue, Fair Haven Community Health Care (FHCHC) has been at the heart of the Fair Haven community. What began as a small clinic operating two evenings a week has grown into a trusted medical home that serves thousands and remains dedicated to providing accessible, high-quality care to all.

The Early Years: Building a Foundation of Care

1971: FHCHC begins with a $5,000 seed grant, offering minor illness care and family planning services to walk-in patients.


1974-1976:  Expands to include comprehensive primary care and prenatal services, with the first baby born in June 1974. Acquires its first building, a Victorian house, establishing a lasting presence on Grand Avenue.


1980s:  Federal funding, the opening of School Based Health Centers, and the introduction of HIV counseling and testing mark significant milestones in expanding our care.

A Legacy of Growth and Innovation

1990s: FHCHC becomes a pioneer in School Based Health Centers, opens multiple locations, and completes the integration of three Grand Avenue buildings. By the end of the decade, FHCHC serves over 35,000 office visits annually.

2000s: The addition of the Wellness Center, diabetes prevention programs, and partnerships with local farms demonstrates our commitment to holistic health.

2016-2017:  Innovations like the E-Consult Program and Refugee Family Clinics, coupled with a new name—Fair Haven Community Health Care—reflect a broadened mission and vision.

Expanding to Meet Community Needs

In 2020, FHCHC launched its first major capital project in over 50 years: the 390 Grand Avenue building. This state-of-the-art 35,560-square-foot facility features:


  • 26 exam rooms for Primary Care and Behavioral Health services.
  • A dedicated focus on Medication-Assisted Treatment for Addiction Medicine.
  • Expanded spaces for the WIC Program, Ryan White HIV/AIDS Services, and a Community Pharmacy.
  • Community spaces, including a training room and a demonstration kitchen for nutrition and wellness education.

Grand Pharmacy

In 2024, Fair Haven Community Health Care opened the Grand Pharmacy to enhance access to affordable, convenient medication for our patients and the Fair Haven community.


The Grand Pharmacy ensures affordable medications with options like HUSKY, Medicare, and a sliding fee scale for uninsured patients. Conveniently located within our health center, it eliminates extra trips while providing personalized support through collaboration with FHCHC clinicians. With a bilingual, culturally sensitive team, we empower patients to confidently manage their health and improve outcomes with same-day services and integrated care.
