You can view your personal health records wherever you have access to an internet-linked computer or smart phone. You can also download the MyChart app for Apple and Android devices.
If it’s your first time and you are setting up your account with your MyChart Access Code click here to sign up. Your MyChart signup access code will appear on the last page of your after visit summary.
You can also request an activation code at any time, by
clicking here
and filling out the online request form. If your email is already on file, you should receive your activation code within 20 minutes.

You are all set! If using Grand Pharmacy, when your prescriptions are ready, you will receive a MyChart notification to let you know it’s time to pick up or schedule free delivery. You may also get notifications for appointment reminders.
