Our Core Care Coordination Services

Our New Haven, CT care coordination team will tailor your services to your unique needs, following a family-centered approach. For example, if you’re experiencing job loss, our team will refer you for job training or financial assistance. On the other hand, if your child is struggling at school, we can assist you with accessing supports and special education services.

Healthy Start

We have a Healthy Start Care Coordinator on our staff. This team member is dedicated to forming an alliance with you to ensure you can have a healthy pregnancy and childbirth experience. Both mothers and fathers are welcome to participate. 

Our Healthy Start program will provide you with the following services to support a healthy pregnancy: 

• Assistance with accessing health insurance.
• Compassionate, caring support to navigate challenges like domestic violence.
• Help with necessities, such as food, diapers, and housing.
• Referrals to medical, dental, and behavioral health specialists. 
• Educational programming related to parenting.
• Connection to fatherhood programs.
