ADDICTION MEDICINE
Our integrated program that provides both Behavioral Health and Medical Care for patients suffering from substance use disorders is available at many of our Fair Haven Community Health Care sites.
ADULT AND GERIATRIC
The Fair Haven Community Health Center provides care to all ages of adults from 18 to over 100. Every adult patient is able to select a primary care provider from one of two distinct clinical teams, which are comprised of physicians and nurse practitioners.
BEHAVIORAL HEALTH
The Behavioral Health Department at Fair Haven Community Health Center believes integrated behavioral healthcare is essential for high quality primary care.
DENTAL SERVICES
Fair Haven Community Health Center’s highly trained dental professionals are happy to offer a full range of preventative and restorative care to children and families.
DIABETES CARE PROGRAMS
Since 2007, the Fair Haven Community Health Center’s Diabetes Prevention Program (FHCHC DPP) has offered 1-hour healthy lifestyle class once per week for participants in the 12-week Intensive Lifestyle Intervention.
FAMILY SUPPORT SERVICES
Our Family Support Services are a key component of our holistic patient care, offering education, resource and counseling services as part of every patient’s good health plan. These services include: Care Coordination, Healthy Start, Nurturing Families Network and Women, Infants and Children (WIC).
INFECTIOUS DISEASE
Fair Haven Community Health Care offers screening and treatment for infectious diseases including HIV/AIDS, tuberculosis, and hepatitis C.
LABORATORY
FHCHC has an on site CLIA-certified laboratory where some laboratory work is performed. Other lab work is sent to private or state laboratories for analysis.
LGBTQ+ CARE
Fair Haven Community Health Care is proud to serve and support the LGBTQ+ community. We are committed to providing compassionate care that is equitable and inclusive.
MIDWIFERY AND WOMEN’S HEALTH
The Department of Midwifery and Women’s Health at the Fair Haven Community Health Center believes in a holistic approach to women’s health.
PEDIATRIC AND WELL CHILD
A team of pediatricians, family physicians, and nurse practitioners provide routine physical exams and management of acute and chronic illnesses in our child-friendly pediatric wing stocked with gently used, donated books.
PHARMACY
We participate in the Federal 340B drug program with numerous area pharmacies. This program lowers medication cost for patients who are uninsured. Commercially covered patients with high prescription co-pays may also be eligible.
SCHOOL BASED HEALTH CENTERS
FHCHC has School Based Health Centers in five New Haven schools including Wilbur Cross High School, Riverside Academy, Fair Haven K-8 School, Clinton Ave School, and John S. Martinez School, and two East Haven Schools: Joseph Melillo Middle School and East Haven High School.
SPECIALTY SERVICES
Your provider may recommend you see a specialty care provider for additional treatment. Fair Haven community Health Care offers an number of specialty care services on site so you don’t have to go elsewhere. Your care team can provide you with more information if required.
WOMEN, INFANTS, AND CHILDREN (WIC)
Fair Haven is proud to participate in the Women, Infants, and Children (WIC) supplemental food program, a state and federally subsidized nutrition-focused program. WIC provides nutrition counseling and helps low-income mothers and their children buy nutritious foods.
