IOP Care Made For Teens

At Fair Haven Community Health Care, we offer IOP services designed specifically for adolescents aged 12 to 18. Our team is here to provide the guidance and care your teen needs to succeed.

Why Choose FHCHC’s Adolescent IOP Services

Our New Haven, CT adolescent IOP is multicultural and child-centered. It is designed to serve children ages 12 to 18.  Adolescents in our program will participate in the following services:


  • Group therapy:  The foundation of our IOP is group therapy sessions. These sessions take place three times per week after school. Children typically participate for 8 to 12 weeks. 
  • Comprehensive assessment: To ensure that our programming is individualized to meet each child’s unique needs, we perform a comprehensive assessment at the start of treatment. 
  • Goal-setting: We’ll work with your child to help them develop goals related to changing negative behaviors, improving school performance, and developing coping skills.
  • Partnership with families: Our services are family-focused. We’ll work with you to improve your child’s functioning and advocate for their needs at school and in the community.
  • Referrals: As appropriate, we’ll refer your child to needed activities within the community.
  • Care Coordination: Connect with behavioral health and community resources through care coordination.


For needs beyond our scope, we’ll ensure your family is referred to the appropriate specialists in the Greater New Haven area.
