Payment Options
Our goal is to make your health care accessible. Fair Haven Community Health Care accepts the following forms of payment:
- Most private health insurance and managed care plans.
- Public health insurance (Medicaid/HUSKY, Medicare).
- Cash, check, credit/debit card.
- Sliding fee scale, based on family size and income.

Payment is expected at the time of your appointment.

Good Faith Estimate
The Federal No Surprise Act protects self-pay or uninsured patients from unexpected health care charges. You have the right to receive a Good Faith Estimate (GFE) of the total expected charges for any non-emergency health care, such as office visits, prescriptions, equipment, medical test, and hospital fees. A GFE is an itemized list of all costs of any non-emergency care or services. A GFE helps patients who don’t have health insurance or aren’t using insurance to “shop around” and compare prices for non-emergency health care.
