[vc_row][vc_column][vc_column_text]Earlier today, Fair Haven Community Health Care, Cornell Scott Hill Health Center and Yale New Haven Hospital jointly announced the creation of a new and innovative partnership which will provide enhanced access to primary care services for the patients we collectively serve.  Several years in the making, we have been able to develop a partnership structure that will benefit each of our organizations and, most importantly, our patients.

Under the proposal, Fair Haven Community Health Care will be working directly with Cornell Scott Hill Health Center and Yale New Haven Hospital to create a joint outpatient care center located at 150 Sargent Drive in New Haven.  It is important to note that this is additional capacity and we will continue to operate all of our existing facilities.

Yale New Haven Hospital will undertake and pay for the capital renovations of the facility and will be transitioning its Primary Care Center from both Howard Avenue and its Saint Raphael campuses to 150 Sargent Drive where both Fair Haven Health Center and the Cornell Scott Hill Health Center will provide primary care. Yale’s Pediatric facility in Hamden will also transition its services to the Sargent Drive location. Yale New Haven will continue to provide strong access to specialty care for all patients and the new site will serve as a teaching site for Yale School of Medicine.

We are very proud to move this initiative forward.  It will allow us to grow and meet the demands of providing exceptional care in a newly renovated setting that will amplify the care we provide in our existing locations.  Collaborating with CSHHC and YNHH will allow for better care coordination across organizations and will have the potential for the development of innovative, new models of patient centered care.
