WELCOME TO MyChart!
What is MyChart?
MyChart, your patient portal, provides easy and private access to your medical information on-line. You can view your personal health records wherever you have access to an internet-linked computer or smart phone. You can also download the MyChart app for Apple and Android devices.
How to sign up for MyChart
If it’s your first time and you are setting up your account with your MyChart Access Code click here to sign up. Your MyChart signup access code will appear on the last page of your after visit summary.
You can also request an activation code at any time, by clicking here and filling out the online request form. If your email is already on file, you should receive your activation code within 20 minutes.
How to set up mobile notifications for MyChart
- Log into your MyChart account.
- Click on your face/initial on the top right hand corner
- Click on the gear icon – Account Settings
- Scroll down to Notifications
- Click on the slider button to turn on
- If you have not connected your phone to MyChart, you will be asked to open your settings, and allow notifications
- Click on Communication Preferences and click the slider button to turn on text message
You are all set! If using Grand Pharmacy, when your prescriptions are ready, you will receive a MyChart notification to let you know it’s time to pick up or schedule free delivery. You may also get notifications for appointment reminders.

Frequently Asked Questions
What can I do through MyChart?
With MyChart, you can:
- Access your laboratory and test results, medical history, appointment information, medications, immunizations and other health information
- Update your personal information
- Request prescription refills
- Request non-urgent appointments with your current providers
- Send and receive non-urgent secure electronic messages with your medical providers- please allow 2 business days for a response
Is MyChart secure?
Can I ask my provider questions on MyChart?
If your have a life-threatening condition, please call 911 or go to the nearest hospital.
Can I request access to my child's account?
Yes. You may request “Proxy Access” to your child’s account. You will need to complete a consent form, found here.
You may also request access for an adult family member, with their consent. The adult form is here.
