The State of Connecticut has launched CTWiz, a site that securely tracks your vaccination records for any vaccines received in Connecticut, as long as your provider submits records to the state (FHCHC does!). Click here to visit the website and find your vaccine records. NOTE: You must register your name exactly as it appears in your providers records.
